Service Coordinator - Compensation: DOE in Salt Lake City, Utah

Our Salt Lake City homecare office is looking for a Service Coordinator to coordinate home care schedules to ensure a high degree of customer service. The Service Coordinator works closely with clinical staff members to guarantee accuracy and timeliness of patient care. In addition, the coordinator is responsible for documenting patient information into the homecare system of record to ensure timely business flow. The coordinator is also responsible for daily scheduling and staffing of client services, gathering and recording initial intake of patient needs, and managing alterations in patient/family requests for change. Requirements: ? High School Degree (or equivalent) required ? One (1) year of experience and/or College Degree preferred ? Current Licensed Practical Nurse (LPN) licensure preferred ? Must be a self-starter with the ability to meet deadlines and excel in a fast-paced team environment ? Must be organized with strong attention to detail ? Strong time management skills ? Leadership and customer service capabilities required ? Public relations ability, interpersonal skills, and professional telephone manner required ? Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be required ?
